Critical Time Intervention (CTI): Evidence-Based Solutions and Support for Housing Stability

While some individuals and families need only a small boost to regain stability, others require coordinated, intensive support to transition from street homelessness to permanent housing. In 2015, Bethesda Cares became one of the first organizations in the United States to implement Critical Time Intervention (CTI) specifically for individuals exiting street homelessness into Permanent Supportive Housing…pioneering a street-to-home model that centers on sustained engagement and continuity of care.

The innovative Critical Time Intervention (CTI) services at Bethesda Cares function as time-limited and evidence-based support to approximately 80–120 individuals annually throughout Montgomery County who have an imminent referral to Permanent Supportive Housing (PSH) through the Coordinated Entry System (CES). CTI is designed to bridge the critical period of transition from homelessness to housing by fostering continuity of care through Housing First activities, increasing housing stability, and improving overall health and well-being.

Rooted in the Housing First model, CTI bridges the critical transition from homelessness to stable permanent housing by promoting continuity of care, increased housing stability, and improved health outcomes. Our Bethesda Cares Permanent Supportive Housing programs maintain a 98% successful housing retention rate with Supportive Housing and CTI services.

Proven Approach, Person-Centered Engagement

CTI Specialists use persistent outreach and relationship-building to engage clients, particularly those with complex needs such as chronic homelessness, serious mental illness, or co-occurring disorders. Our experience confirms that clients are more likely to participate in the housing process when permanent housing feels attainable, not theoretical…resulting in higher engagement and faster progress.

CTI includes three phases over 3–9 months:

•          Pre-CTI/Transition – Intensive support as clients move off the streets and out of shelters into permanent housing.

•          Try-Out – Reinforcing supports and encouraging client-led problem-solving.

•          Transfer of Care – Connecting clients to long-term supports as CTI staff step back.

Housing-Focused, Outcome-Driven Services

Once housing options are available, CTI Specialists assist clients in:

•          Completing subsidy paperwork and leasing activities

•          Coordinating behavioral and primary health care

•          Strengthening support networks

•          Adjusting to new living environments

•          Building skills for tenancy and community integration

Staff conduct home visits and follow-up, collaborating with community providers to promote self-sufficiency, health access, and housing retention.

Measurable Results

Internal program tracking consistently shows that CTI participants:

•          Achieve higher rates of housing placement and retention

•          Report better mental health satisfaction and reduced emergency system use

•          Develop lasting connections to community support

These outcomes align with national benchmarks set by SAMHSA and the U.S. Interagency Council on Homelessness, confirming CTI’s role in advancing housing stability and overall well-being.

Let’s Go!

In 2016 we expanded our CTI program with a new initiative—the Let’s Go! Program. Each month we invite all our formerly homeless clients out for a day trip—something like a baseball game at Nationals Stadium, mini golf, bowling or fishing at a nearby park. Since we began this program, dozens of our clients have gotten out into the community, formed new friendships, and enjoyed some well-deserved interaction with others.
